Documentation Guidelines
Documentation is confidential information from a qualified professional (e.g. a therapist, doctor, rehabilitation counselor, audiologist, nurse practitioner, or mobility specialist) who is knowledgeable about your disability. We want to understand the relevant impacts of your disability on common tasks that will be part of your academic and campus experience to determine appropriate accommodations and facilitate access.
Please note that you should not delay meeting with us out of concern for not having the right paperwork. Our first priority is meeting with you.
Acceptable documentation will include the following:
- Student name
- Provider name & title, clinic/practice name
- Date
- Diagnosis
- Impact of the disability within the Minnesota State University, Mankato environment
- Recommendations for accommodations
Documentation may include, but is not limited to:
- Psychoeducational evaluations
- Neuropsychological assessments
- 504 plans
- Individualized Education Plans (IEP)
- Letter from health providers
- Information on previous use of accommodations
- Military service-connected (disability) paperwork
The following templated forms may be used by the professional you are working with to obtain the appropriate documentation. Please verify the professional completed the form by including a clinic fax sheet, physician stamp, or cover letter.

Completed documentation can be provided in the following ways:
- Upload it to the MavAccess portal when completing the application.
- Email it to ar@mnsu.edu.
- Fax it at 507-389-1199.
- Bring a paper copy to Accessibility Resources (Memorial Library 132).
