MDE Form - Spring Semester

The MDE form, also known as the Notice of Student Registration form, is required for all students, each semester. This form is not required to be submitted with your application but is required prior to being enrolled into orientation.

State law requires students to speak with their high school by May 30th if you have an interest in enrolling in PSEO courses the upcoming year.
